This is found in a golden chest, within the sewers beneath Lut Gholein and guarded by a mob called Radament. You will first need to find the Horadric Scroll to activate the Horadric Staff quest, by returning said scroll to Deckard Cain. Complete Act I before your search for the Horadric Cube begins. This means it isn't an optional quest like rescuing Deckard Cain, which means you will need to kill Andariel first.
